Telephonic outreach and customer engagement, support and assistance
Support and coordination of complex situations - leveraging community resources
Educate and refer customers to available social and health resources,
Consultation with Cigna Medical Management teams and Cigna Behavioral Health teams and programs as appropriate
Identifies and educates appropriate client specific programs
Empowers customers with skills to provide enhanced interaction with their providers
Documents customer interactions and interventions as directed
Consult with and assist team members in cases where a customer's medical and behavioral issues are impacting their ability to set and/or achieve goals
Collaboration with team members such as nutritionist, pharmacist, case management nurses, specialty team member, Medical Director and other internal partners across Cigna's programs as appropriate
Provide ongoing consultation and training to the team on relevant social topics, resources available
Provide connection and assistance to agencies where alternative funding may be needed (i.e. SSI, SSDI benefits, state victim assistance, VA state funded rehab or community resources
Life planning needs such as end of life, home/financial management and accessible housing
Resources for community resources for conditions and bereavement
Assumes lead role with customer when appropriate but integrates with warm connection with Cigna team members to support ongoing care and needs
Qualifications
Licensed Clinical Social Worker (LCSW)
A minimum of five (5) years clinical experience in behavioral health/social work field
Experience with crisis intervention and psychosocial assessments
Experience in health education and wellness coaching preferred
Experience with direct customer communication (written and verbal), experience in telephonic counseling/coaching required
Working knowledge of motivational strategies, training and consultation required
Thorough knowledge and understanding of health behavior change theories and their application
Proven administrative abilities, with strong computer and software application skills
Excellent interpersonal skills and the ability to work in a team environment
Demonstrated ability to set priorities
A high energy level and excellent written and oral communication skills are essential
Passion for health improvement of customers
Preferred Competencies
Excellent time management, organizational, research, analytical, negotiation, communication (oral and written) and interpersonal skills
Strong personal computer skills, MS word, Excel, Outlook experience, and Internet research desired
Knowledge of the insurance industry and claims processing
Service marketing/sales and business acumen experience a plus
Strong skills in the following areas: teamwork, conflict management, assessment complex issues, ability to recommend changes and resolve problems through effective decision making
Experience in medical management and case management in a managed care setting is highly desirable
Knowledge of managed care products and strategies
Demonstrated sensitivity to culturally diverse situations, customers and clients
Ability to work within changing business environment and balance business needs with patient advocacy
Experience managing multiple projects in a fast paced matrix driven environment
Demonstrated ability to coach and educate other case management staff
Demonstrate effective organizational skills and flexibility to meet the business needs
