Medication Distribution and Control
a.    Medication orders: Shall review all medication orders and assess the completeness and appropriateness of the medication profile prior to dispensing and administration.  Except in the case of a true medical emergency, medication may not be dispensed from the pharmacy without documentation of the patient’s drug allergy status.  Allergy information must be part of the patient information profile of the pharmacy computer system. 
b.    Formulary maintenance: Shall be responsible for maintenance of drug formulary policies and procedures, including completion of all required forms and documentation.
c.    Extemporaneous compounding: Able to prepare all required drug formulations, dosage forms, strengths as required.
    MEDICATION ORDER ENTRY
1.    Routinely enters all required patient demographic information, including allergies, height and weight
2.    Able to enter medication orders to include:
*        Generic drug
*        Strength
*        Route
*        Directions
*        Start/Stop Dates
*        Checks all pediatric dosing and enters   mg/kg in comment section of order       entry
3.     Verifies all technician computer entries for accuracy, completeness and appropriateness.
4.     Maintains order entry error rate 
< 0.5%.
5.     Able to generate all required reports: MAR, drug usage, food-drug interaction, etc. necessary for daily review.

*    Routinely inspects all stocks of medications to ensure the absence of outdated, unusable or mislabeled products. 
*    Assures proper storage conditions.

2.    Adverse drug reaction identification, reporting and prevention: 
*    Able to appropriately participate in the hospital’s ADR Reporting Program.

Qualifications :
Staff Pharmacist Per Diem Days candidate will possess the following education, licenses/certifications, and experience:
A.    Minimum Education: Graduate of a college of pharmacy accredited by the American Council on pharmaceutical education (ACPE).  Doctor of pharmacy degree preferred.
B.    Minimum Experience:  Two years of hospital experience preferred.  Hospital pharmacy residency preferred.
C.    Preferred Skills and Abilities:  Must possess a specialized pharmaceutical knowledge in addition to a good working knowledge of pharmacokinetic dosing and nutritional support.  Effective verbal and written communication skills.  Computer working knowledge.
Skills and Proficiencies Specific to the Position
1.    Possesses the skills and knowledge to provide all aspects of pharmaceutical care for all PEDIATRIC patients, including calculating medication dosages, monitoring the effects of medication and outcomes of therapy;
2.    Possesses the skills and knowledge to provide all aspects of pharmaceutical care for all ADOLESCENT patients, including calculating medication dosages, monitoring the effects of medication and outcomes of therapy;
3.    Possesses the skills and knowledge to provide all aspects of pharmaceutical care for all GERIATRIC patients, including calculating medication dosages, monitoring the effects of medication and outcomes of therapy;
4.    Possesses the skills and knowledge to provide all aspects of pharmaceutical care for all ADULT male and female patients, including calculating medication dosages, monitoring the effects of medication and outcomes of therapy.
