Cedars-Sinai's **Emergency Department (ED),** with 51 patient beds and 7-space Fast Track is a Level I Trauma Center, Level II Pediatric Trauma Center, Pediatric Critical Care Center, Level III Geriatric Care ED, STEMI Receiving Center, Stroke Receiving Center and Paramedic Base Station. The ED is open 24 hours a day, seven days a week available to all people in need. The Emergency Department (ED) Community Resource Coordinator will work in collaboration with ED Leadership and the clinical care team which consists of case managers, social workers, physicians and nurses to identify and assist unsheltered homeless patients within the hospital and post discharge. The ED Community Resource Coordinator acts as the liaison between ED staff and the patient to assist the patient in optimally navigating the health system, delivering health education, and serving as a patient advocate thereby encouraging the patient healthcare partnership. **Primary Duties and Responsibilities:** + Provide a positive link between the ED, patient, their support group, other members of the ED Team and community supports. + Promote patient satisfaction and happiness by ensuring the well-being of patients by attempting to fulfill their needs, while reducing the stress on the patient. + Conduct individualized needs' assessment for unsheltered patients and work with patients to establish goals. + Provide information, referrals, linkages, and advocacy to assist clients in accessing services and resources. + Maintain collaborative relationships and work closely with community and county-based organizations and homeless services to ensure seamless transitions of care for patients. + Assist unsheltered patients with accessing resources and making referrals; continue to engage with unsheltered patients until adequate resource linkage is acquired. + Provide case management services for unsheltered patients to facilitate post-discharge appointments and connect them with services for mental health, housing, substance recovery, advocacy, socialization activities and other services. + Provide basic psychological/emotional support and crisis intervention to unsheltered patients. + Maintain patient documentation, care plans and other reports as vital. **Educational Requirements:** + A High School Diploma or GED is required. + Associate degree or above preferred. **Experience:** + Minimum of 1 year recent experience working with homeless and other vulnerable populations, engaging with patients and/or experience in care coordination/case management, medical office, community outreach, social work or customer service. + Experience conducting assessments of general patient information, developing short-term care plans, and providing vital interventions identified during assessments. + High standard of proficiency using Microsoft Windows and other Microsoft Office software: MS Excel, Outlook, Powerpoint Word, etc. + Knowledge of community resources in Los Angeles, especially for homeless and underserved populations and, familiarization with community-based health care delivery. + Knowledge of the HMIS and CES system preferred. + Demonstrated combination of training, experience, problem solving skills and background that involves customer service. + Ability to work with diverse populations, especially underserved members of the community including those suffering from mental health disorders and substance abuse. + Ability to convey and/or receive written/verbal information to/from various audiences in different formats. **Working Title:** Community Resource Coordinator - Emergency Department - 10-Hour Days **Department:** Emergency Rm Admin **Business Entity:** Service Lines & Ops **Job Category:** ,Patient Services,Patient Services **Job Specialty:** Patient Relations **Position Type:** Full-time **Shift Length:** 10 hour shift **Shift Type:** Day Cedars-Sinai is an EEO employer.
