Inpatient Care Coordinator II - Part Time

This position supports the inpatient case manager to create a seamless transition for hospitalized patients and facilitate customer satisfaction. Documents in electronic medical record systems as directed. Focus should be on patient discharge from hospitals into lower level of care by providing the patient with community and alternative solutions.
Job Responsibilities/Duties
Reviews daily hospital census and faxes and enter any new admissions into Essette and notifies inpatient case manager of admission prior to 9:50am OR Reviews daily hospital census and calls hospitals for discharge dates, enter discharge date into Essette prior to 9:50am
Enters all referral requests and approves referrals that are on the "Approval" list for coordinators.
Documents all pertinent information in IDX or Essette.
Uploads all Hospital Documents post discharge into IDX or Essette.
Manages the inpatient call queue (high volume).
Manages work voice mail and responds to messages within 24 hours.
Notifies the discharge coordinator of discharges.
Discharge planning- call hospitals, finding home health, SNF, DME, specialist and authorize services as appropriate with CM
Perform appointments scheduling for PCP and specialist
Continue to manage fax queue throughout the day
Assist with setting up temporary contracts for new providers by obtaining all necessary information and forwarding to the contracting department by email or via the 'pending contracting" queue in IDX.
Facilitate referrals for patient education to appropriate internal resources (health education, disease management programs, etc.)
Review SNF Coordinator NONMC Audit Logs
Assist with special projects as assigned by management.
Qualifications
Minimum Education: High school diploma or equivalent required. This position is entry level and will be trained.
Minimum Experience: Customer Service or Medical Experience preferred. HMO/Medical Group experience preferred.
Req. Certification/Licensure: Certified Medical Assistant preferred. Medical Terminology certificate preferred.
